What Is a Consent Form? Why It Matters — And Why So Few People Actually Understand What They're Signing
Summary
Informed consent is supposed to be the moment a research participant fully understands what they're agreeing to. In practice, it rarely works that way. Fewer than 10% of participants report reading an entire consent form before signing it,⁵ and in controlled comprehension tests, only 28% of participants could correctly answer basic questions about what they'd just consented to.⁵ The gap is widest for the communities research is most supposed to serve: people with lower health literacy — disproportionately racial and ethnic minorities, older adults, and people with less formal education — are the group most likely to struggle understanding a consent document in the first place.⁷ This post breaks down what a consent form actually is, why comprehension (not just signature) is the real ethical bar, what the data says about who gets left behind, and how research partners like Lumovance help organizations close that gap.

What Is a Consent Form?
A consent form — more formally an informed consent document (ICD) — is the written agreement a research team gives a potential participant before they join a study. Informed consent is considered a cornerstone of research involving human subjects, meant to uphold a participant's autonomy and protect them from harm.¹ At minimum, it's supposed to explain what the study involves, what risks and benefits exist, how personal data will be used, and that participation is voluntary.
The problem is what these documents actually look like on the page. Most informed consent forms are long, legalistic, and written at an 11th-grade reading level² — a level that assumes a degree of formal education a large share of the U.S. adult population doesn't have. One clinical trial registry analysis found that only about 6% of consent forms and participant information sheets are written below an eighth-grade reading level, even though that's roughly the level health literacy experts recommend for public-facing documents.³
Why Is a Consent Form Important to Understand?
A signature only means something if the person understands what they signed. That distinction — informed versus merely documented — is where research ethics actually lives.
The data suggests that distinction is breaking down. In one study measuring what participants retained immediately after signing, scores on a six-item comprehension assessment averaged just 4.66 out of 6, and only 3 of 73 participants had a perfect score.⁴ Risk information — arguably the most important thing a form is supposed to convey — was the weakest area: 53% of participants were not aware of two or more of the study-related risks they had just agreed to.⁴
This isn't a fringe finding. Even when forms were shortened to meet federal Common Rule guidance, participants on average understood only about half of the information contained in those shortened versions.⁶
What If I Don't Read the Consent Form?
This is a more common outcome than most people assume — and it's not just a matter of not reading closely. In one study, fewer than 10% of participants reported reading the entire consent form, and across two experiments testing ways to get people to slow down, the proportion of participants who correctly followed embedded instructions in the form never exceeded 45%.⁵
Skipping the form doesn't just create an ethics gap — it has practical consequences for participants. Someone who didn't absorb the risk section may not recognize a study-related symptom when it appears. Someone who missed the data-use section may be surprised later about how their information is shared.
Who's Most Affected? The Health Literacy Gap Isn't Evenly Distributed
This is where comprehension becomes an equity issue, not just a design issue.
National Assessment of Adult Literacy data shows that just 12% of U.S. adults have "proficient" health literacy — enough to confidently navigate a document like a consent form. The majority, 53%, fall into the "Intermediate" category, while 22% are "Basic" and 14% are "Below Basic."⁸ That's tens of millions of adults for whom a standard consent document is, functionally, not written in a language they can fully use.
That gap tracks closely with the communities research equity work is trying to reach:
Lower health literacy is more prevalent among racial and ethnic minorities, older adults, and people with less formal education.⁷
Black American and Latino participants together represent fewer than 10% of patients enrolled in clinical trials nationally, according to the National Institute on Minority Health and Health Disparities.⁹ Poor understanding of clinical trials, low health literacy, and confusing enrollment procedures are documented barriers alongside medical mistrust.¹⁰
Consent forms translated into a participant's native language can improve satisfaction and enrollment — but many research programs don't have the resources to provide that translation, quietly excluding non-English-dominant communities from studies meant to represent them.¹¹
Justice-impacted adults face compounding barriers: low health literacy among this population is described as a foundational and costly challenge to accessing care.¹²
What Actually Fixes This? The Evidence on Better Consent Design
The encouraging finding across this research is that comprehension isn't fixed — it's a design problem, and design problems have solutions.
A recent randomized trial comparing long, medium, and short consent forms found that a medium-length form written at a sixth-grade reading level, with shorter sentences, tables, and better formatting, produced significantly better comprehension than either the long or short versions — and the short form performed no better than the long form typically used in approved clinical trials today, meaning length alone isn't the fix.² Format and reading level are what move the needle. Separately, requiring a fixed reading time and embedding a short comprehension quiz increased engagement, and live and audiovisual delivery formats improved both comprehension and perceived readability compared to standard written text.⁵ ⁶

Right now, most participants don't read the full form, most U.S. adults don't have the health literacy to fully navigate one even if they did, and the gap is widest among the exact communities equity-focused research is trying to reach. The fix isn't complicated — plain language, better formatting, and testing with real participants — but it requires treating consent design as a deliberate part of the research process, not an afterthought.
